Bible Answer

What scriptures can guide someone questioning their gender and considering surgery?

What scripture can be used to minister to someone who has questions about being transgender and may be seeking surgery?

The issues of transgender identity and homosexuality have sparked intense debate within modern culture and society, particularly among evangelical groups. Many Christians struggle with how to uphold a biblical perspective on sexuality in a world that increasingly embraces evolving definitions of gender and biological identity.

In contemporary discourse, the LGBTQ+ community has influenced discussions on sexuality, advocating for an individual's right to determine their sexual orientation and gender identity. However, rather than embracing God’s sovereign design of male and female, society has placed this decision in human hands, rather than in the hands of the Creator. As Romans 1 states:

ROM. 1:25 They exchanged the truth of God for falsehood and worshiped and served the creature rather than the Creator, who is blessed forever. Amen.

Despite modern discussions, the struggle with gender identity is not new. The Roman poet Ovid illustrates this in the myth of Iphis and Ianthe, where Iphis, born female, was raised as a male due to societal pressure. This historical context shows that such discussions have persisted for centuries.

Ministering to Those with Gender Identity Questions

A Christian approach to individuals questioning their gender must be rooted in compassion, as every person is made in the image of God. The first question to consider is whether the individual is a believer in Christ. This distinction determines how a Christian should respond.

For non-believers, holding them to biblical standards without a foundation in Christ is ineffective, as their hearts remain unchanged. John 3:19-20 affirms this reality:

JOHN 3:19 This is the judgment, that the Light has come into the world, and men loved the darkness rather than the Light, for their deeds were evil.
JOHN 3:20 For everyone who does evil hates the Light, and does not come to the Light for fear that his deeds will be exposed.

Before discussing specific sins, a non-believer must first hear and understand the Gospel, realizing their need for salvation as Romans 3 clearly warns:

ROM. 3:10 as it is written,
“There is none righteous, not even one;
ROM. 3:11 There is none who understands,
There is none who seeks for God;
ROM. 3:12 All have turned aside, together they have become useless;
There is none who does good,
There is not even one.”

Only through faith in Christ can an individual be forgiven, transformed, and empowered by the Holy Spirit to resist sin’s hold. This is the hope offered to all who come to Christ.

Addressing Gender Reassignment Among Believers

If a person struggling with gender identity professes faith in Christ, fellow believers must approach them with love and biblical truth. Psalm 139 reminds us of God’s perfect design:

PS. 139:13 For You formed my inward parts;
You wove me in my mother’s womb.
PS. 139:14 I will give thanks to You, for I am fearfully and wonderfully made;
Wonderful are Your works,
And my soul knows it very well.
PS. 139:15 My frame was not hidden from You,
When I was made in secret,
And skillfully wrought in the depths of the earth;

God does not make mistakes in His creation. To seek gender reassignment is to question the Creator’s perfect design. Such struggles stem not from God’s error, but from the fallen nature of humanity introduced in Genesis 3. All sin, including gender confusion and same-sex attraction, arises from this brokenness. Scripture calls believers to embrace their identity in Christ and reject sinful inclinations. Romans 6:11-14 exhorts:

ROM. 6:11 Even so consider yourselves to be dead to sin, but alive to God in Christ Jesus.
ROM. 6:12 Therefore do not let sin reign in your mortal body so that you obey its lusts,
ROM. 6:13 and do not go on presenting the members of your body to sin as instruments of unrighteousness; but present yourselves to God as those alive from the dead, and your members as instruments of righteousness to God.
ROM. 6:14 For sin shall not be master over you, for you are not under law but under grace.

True faith in Christ requires surrendering our desires to His truth, even when they conflict with personal feelings. For the scripture let us know that our hearts (feelings) are deceitful and sick without complete reliance and surrender to God and His word: As Jeremiah 17:9 warns:

JER. 17:9 “The heart is more deceitful than all else
And is desperately sick;
Who can understand it?

We must trust in God’s Word over our emotions. Jesus Himself demonstrated this by resisting temptation in the wilderness, relying on Scripture and submission to the Father’s will.

The Role of Accountability in Christian Community

Believers must be willing to lovingly hold one another accountable to Scripture as scripture contends:

PROV. 27:6 Faithful are the wounds of a friend,
But deceitful are the kisses of an enemy.

Encouraging a brother or sister to align their life with God’s truth is an act of love, not judgment. Hebrews 12:5-11 reminds us that God disciplines His children for their good, shaping them into holiness. Though discipline may be painful, it ultimately produces the peaceful fruit of righteousness:

HEB. 12:5 and you have forgotten the exhortation which is addressed to you as sons,
“My son, do not regard lightly the discipline of the Lord,
Nor faint when you are reproved by Him;
HEB. 12:6 For those whom the Lord loves He disciplines,
And He scourges every son whom He receives.”
HEB. 12:7 It is for discipline that you endure; God deals with you as with sons; for what son is there whom his father does not discipline?
HEB. 12:8 But if you are without discipline, of which all have become partakers, then you are illegitimate children and not sons.
HEB. 12:9 Furthermore, we had earthly fathers to discipline us, and we respected them; shall we not much rather be subject to the Father of spirits, and live?
HEB. 12:10 For they disciplined us for a short time as seemed best to them, but He disciplines us for our good, so that we may share His holiness.
HEB. 12:11 All discipline for the moment seems not to be joyful, but sorrowful; yet to those who have been trained by it, afterwards it yields the peaceful fruit of righteousness.

Ministering to those struggling with gender identity requires both grace and truth. For non-believers, the primary goal is to share the Gospel, as transformation begins with salvation through Christ. For believers, encouragement to embrace God’s design and trust in His sovereignty is key. Scripture is clear: God’s creation is intentional, and true identity is found in Him. By keeping a high view of God and submitting to the Spirit, believers can resist sin and walk in obedience. Ultimately, Christ alone offers the power to overcome sin and the promise of new life.

Furthermore, we must remember that prayer is essential for spiritual warfare. Reliance on God through prayer when sharing the Gospel or addressing a brother or sister’s sin is a must. If we are not surrendered to Him, we are not ready for such conversations.
